Parents/Guardians of Fayetteville Elementary Students,
With the ongoing transition to Final Forms we felt it is crucial to make sure we have accurate information. For this reason, we are requesting that this form be completed by all elementary parents in addition to information that is in Final Forms. It is important that we have correct information regarding your instructions for the transportation of your child before and after school hours. We understand that circumstances for each family may be different from day to day over the course of the week. We have developed this form so we can pick up and drop off our students safely and efficiently each day. We ask that you fill out and sign this form every year. If it is necessary to make changes during the school year please call the elementary office. Office personnel will use the prior form until an updated form is submitted. If you have any questions please call the elementary office at (513) 875-2083. EACH INDIVIDUAL STUDENT MUST HAVE THIS FORM ON FILE SO FILL OUT THIS FORM FOR EACH OF YOUR CHILDREN WHO ARE STUDENTS AT THE ELEMENTARY.
